A homeopathic experiment with snake venom on human immunodeficiency virus (HIV) has given rise to excitement among scientists to work on a possible relief to AIDS and Ebola patients, according to Global Homeopathy Foundation (GHF) managing trustee Sreevalsa Menon.

GHF, founded by a group of homeopathic activists, is a non-government non-profit making organisation.

Research by doctors at the Hyderabad-based JSPS Government Homeopathic Medical College and Indian Institute of Chemical Technology (IICT), it is stated to have shown that homeopathic medicine from snake venom, Crotalus Horridus, can arrest the multiplication of HIV.

In another two-year long study, Mumbai-based homeopath Rajesh Shah has developed a new medicine for AIDS patients, sourced from HIV itself. The drug has been tested on humans for safety and efficacy and the results are encouraging. Shah’s scientific paper for debate has just been published in Indian Journal of Research in Homeopathy (IJRH), the official publication of the Central Council for Research in Homeopathy (CCRH) in its online edition.

These and other research studies will be presented at the two-day World Homeopathy Summit to be held at Mumbai from April 11. Scientists and doctors from across 25 countries have confirmed their participation at the summit being organised by GHF, Menon stated in a press release here on Thursday.

“Our experiment entails that the homeopathic drug has the capacity to act on HIV, etc,” said Praveen Kumar, head of Department of Practice of Medicine at JSPS College. He, however, pointed out that “it is too early to declare anything big, but our work has certainly opened the floodgates of advanced research and clinical testing.”

Prathama S Mainkar, fellow QRS Division of Natural Products Chemistry at IICT, said her team experimented with homeopathic dilutions as well but found that the medicine made out of snake venom was the most useful.

For years, homeopathy is stated to have been using the process of converting snake venom and poison from scorpions, spiders and wild bees into medicinal substances by transforming them into nano-particles that have proved safe and effective for patients.

GHF has invited conventional doctors as well to participate in the meet and update themselves on the “significant” research that has been going on for the past 15 years, Menon said.

The summit will also focus on the need to integrate both the systems of medicine to meet challenges posed by various diseases because mono-therapy may not work for all patients, he added.

Johnson & Johnson and partner GlaxoSmithKline PLC subsidiary ViiV Healthcare, which specialises in anti-retroviral drugs, announced yesterday encouraging initial trial results on a drug that could revolutionise HIV treatment.

Glaxo said the results from the study showed the investigational, long-acting, injectable combination of Cabotegravir, ViiV Healthcare’s treatment, and Rilpivirine, made by Johnson & Johnson’s Janssen unit, were highly effecting in maintaining viral suppression rates.

A single injection to a patient once every eight weeks has a higher viral suppression than patients on tablets, revealed the trial result.

The results from the phase IIb of the first 32 weeks of the planned 96-week study, which combines one drug from each company, showed very encouraging results.

In the trial 309 patients tested first all had prior treatment with daily pills that reduced the HIV virus to undetectable levels in their blood. About 95 percent of those who then got the experimental injections had the bloodborne virus kept in check over the 32 weeks, versus 91 percent in a comparison group taking three pills each day. The two groups continue to receive medication and periodic testing of their blood.

More trials and research will be conducted but Paul Stoffels, ViiV’s chief scientific and medical officer, believes it could be on the market within the next five years. If the result is confirmed in larger final-stage trial, the combination could be available by 2020.

He said: “ViiV Healthcare is committed to identifying new therapeutic options for physicians and people living with HIV. These initial phase IIb data investigating long-acting cabotegravir and rilpivirine are promising and build on the results we have seen to date. We look forward to seeing further results as we move into phase III.”

There has been talk of a HIV cure or vaccine for many years, but after three decades, we are no closer to either, but this drug could revolutionise treatment of HIV.

Medication doesn’t work for everyone, side-effects can be problematic and adherence can be a struggle for many, especially newly diagnosed patients and this new drug could be the answer to these issues and have an extremely effective suppression rate.

Of course not everyone with HIV will be able to take this combination due to other medical conditions and possible side-effects, but after the success of these initial trials, the companies are already planning to develop other combinations. An injection every eight weeks instead of daily pills could mean stronger protection and a decrease in drug-resistance.

Another possible use for this could be for HIV prevention. In some countries, PrEP (a once a day pill) is currently being used by HIV negative people as protection from HIV. If an injection once every eight weeks could be administered in the same way to negative people, this could dramatically decrease the amount of new infections every year.

These initial results look promising and if the larger trials are successful, this could change the lives of millions all over the world.

The results of the clinical trial, published in Nature, have been more successful than previous HIV antibody tests in humans, with the researchers finding that their experimental therapy can reduce the amount of virus present in a patient's blood significantly.

"What's special about these antibodies is that they have activity against over 80% of HIV strains and they are extremely potent," says co-first author Marina Caskey, an assistant professor of clinical investigation in the Nussenzweig Laboratory of Molecular Immunology at the Rockefeller University, New York, NY.

The antibody in question is 3BNC117, and it has shown activity against 195 of the 237 HIV strains. 3BNC117 works by targeting the CD4 binding site of the HIV viral envelope; the CD4 receptor is the main site of attachment of HIV to host cells.

Around 10-30% of humans with HIV naturally produce broadly neutralizing antibodies, but these only tend to develop several years after infection. During this time, the virus is able to evolve, changing shape to protect itself from the antibodies.

Isolating and cloning these antibodies allows the researchers to deliver them instead as therapeutic agents, administering them against HIV infections before the virus is able to evolve. This process is known as passive immunization.

The antibody showed success when used in mouse and nonhuman primate models of HIV, leading the team to commence human trials.

For the study, the researchers tested four different doses of 3BNC117 on a total of 29 participants. Each was given a single dose of the antibody and monitored for the next 56 days. Of these participants, 17 had HIV and 15 of these participants did not take any antiretroviral medication for the duration of the study.

Among the eight participants that received the highest dose of the antibody - 30 mg per kg of weight - the researchers found that the amount of virus in the blood was reduced by 8-250 times for a period of 28 days.

A single dose of the antibody was well tolerated by each participant and was successful at temporarily reducing viral loads. The viral loads of some participants remained below starting levels even at the end of the study period.

However, the researchers noted that in two of the participants receiving the highest dose of 3BNC117, the antibody became approximately 80% less effective in neutralizing the virus after 28 days of treatment. This reduction may have been due to the virus evolving in order to evade the antibody.

"One antibody alone, like one drug alone, will not be sufficient to suppress viral load for a long time because resistance will arise," says Caskey. Like many other antiretroviral drugs, it is likely that 3BNC117 will need to be utilized in combination with other antibodies or drugs.

The research group have developed a second HIV antibody and plan to test it both alone and in combination with 3BNC117 in the future. "The goal is a once-a-year shot for prevention and a combination approach for cure," states lead author Michel Nussenzweig, an infectious disease physician and immunologist at the Rockefeller University.

In addition to these findings, the study also raises hopes for a future HIV vaccine. By inducing an uninfected individual's immune system to create these kinds of antibodies, researchers could prevent HIV infections before they take hold.

"In contrast to conventional antiretroviral therapy, antibody-mediated therapy can also engage the patient's immune cells, which can help to better neutralize the virus," says co-first author Florian Klein, also an assistant professor of clinical investigation in the Nussenzweig laboratory.
